At Penhaven Maternity Unit, Dr Saira Roskilly has one rule: never let comfort sound like a promise. After years in tertiary neonatal care, she has returned to Cornwall determined to make every difficult conversation exact.

Rhys Calder has a different rule. As an advanced neonatal nurse practitioner and regional retrieval coordinator, he knows families remember what happens after the handover just as much as what was said before it. He tells the truth, too - but he stays.

When Ivy Polmear is born far too early for antenatal transfer, Saira and Rhys are drawn into repeated collaboration through stabilisation, transfer, training, repatriation and the uncertain road toward home. Their clinical instincts fit almost perfectly. Their beliefs about what care requires do not.
