Arriving at the appropriate diagnosis is among the most important tasks in modern medicine. Yet errors are common - 5-15% of diagnoses across diverse care settings are incorrect. Diagnostic error is a leading cause of preventable harm and the most frequent reason for malpractice claims. The purpose of this book is to provide tools for thoughtful diagnostic reasoning and error reduction. We begin with an overview of commonly misdiagnosed serious conditions, followed by strategies for constructing thorough differential diagnoses and avoiding diagnostic pitfalls. The bulk of the text is then organized by presenting symptom. For each symptom, we provide structured checklists of "do-not-miss" diagnoses, common causes, and commonly misdiagnosed conditions. Each section also includes systematically-organized differential diagnoses, "red flag" associated symptoms, and other essential considerations. More than 80 of the most common presentations are covered, along with related and overlapping symptoms.
